Job Summary

A company is looking for a Care Manager, RN.

Key Responsibilities

• Develop, oversee, and ensure continuity of patient care plans
• Participate in interdisciplinary team meetings and manage healthcare costs
• Review and revise plans of care to meet regulatory standards and compliance

Required Qualifications

• Registered Nurse licensure required
• Two years of home care field experience preferred
• One year of clinical review preferred
• Two years of case management and/or utilization management experience preferred
• Proficiency in HCHB preferred
