Job Summary

A company is looking for a Registered Nurse Care Manager.

Key Responsibilities

• Conduct comprehensive assessments of patients' medical, behavioral, pharmaceutical, and social needs
• Create and implement care plans in collaboration with patients and the care team to address identified needs
• Coordinate care across various settings and provide education and self-management support to patients

Required Qualifications

• 1+ years of nursing experience in case management or care management
• 2+ years of healthcare-related experience
• Current, unrestricted compact Registered Nurse license
• Valid BLS certification from a licensed AHA or American Red Cross training facility
• Core values consistent with a patient-centered approach to care
