Job Summary

A company is looking for a Clinical Program Coordinator RN for a remote position.

Key Responsibilities

• Provide care coordination services, including disease management and case management for members with complex health needs
• Assist members with managing health plan benefits and finding alternative services when needed
• Educate and empower members to manage their health conditions effectively

Required Qualifications

• 5 years of clinical nursing experience
• Current unencumbered Registered Nurse License in state of residency upon hire
• Experience collaborating with physicians in patient care management
• Bachelor's Degree in Nursing or health education is preferred
• Current nursing experience in areas such as cardiology, endocrinology, pediatrics, obstetrics, oncology, or respiratory health is preferred
