Quality Clinical Coordinator

Job Summary

A company is looking for a Quality Clinical Coordinator to support patient outreach and clinical quality initiatives.

Key Responsibilities

• Conduct proactive outreach to patients to address medication adherence barriers and encourage timely refills
• Collaborate with healthcare professionals to coordinate follow-up care and resolve medication access issues
• Manage patient lists and document interactions across multiple platforms, ensuring accuracy and compliance

Qualifications and Skills

• Previous experience in a healthcare setting such as a physician's office, hospital, pharmacy, or health plan
• Proficiency with electronic medical records (EPIC required; Salesforce, Arcadia, and eCW experience a plus)
• Familiarity with medication adherence programs, Stars/HEDIS measures, and managed care is highly valued
• A degree or medical certification (such as Medical Assistant) is preferred, though equivalent hands-on experience will be considered
