A company is looking for a Quality Clinical Coordinator to support patient outreach and clinical quality initiatives.
Key Responsibilities
- Conduct proactive outreach to patients to address medication adherence barriers and encourage timely refills
- Collaborate with healthcare professionals to coordinate follow-up care and resolve medication access issues
- Manage patient lists and document interactions across multiple platforms, ensuring accuracy and compliance
Qualifications and Skills
- Previous experience in a healthcare setting such as a physician's office, hospital, pharmacy, or health plan
- Proficiency with electronic medical records (EPIC required; Salesforce, Arcadia, and eCW experience a plus)
- Familiarity with medication adherence programs, Stars/HEDIS measures, and managed care is highly valued
- A degree or medical certification (such as Medical Assistant) is preferred, though equivalent hands-on experience will be considered
