Arizona Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate care
• Coordinate with providers and community resources to ensure members receive person-centered care
• Monitor member progress and update care plans as necessary to meet individual goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Must reside in a state within the Mountain Time Zone
• Standard work hours are 8:00 AM - 5:00 PM MT
