Job Summary

A company is looking for a Care Coach I to empower caregivers through support and education.

Key Responsibilities

• Engage and educate caregivers using phone and digital technology to enhance their well-being
• Provide support for caregiver programs, focusing on resource navigation and care coordination
• Document coaching interactions and identify caregiver challenges to provide appropriate resolutions

Required Qualifications

• 2+ years of experience in care/disease management with complex populations
• Knowledge of Medicaid, Medicare, and Managed Care systems
• Strong understanding of case management and community service delivery systems
• Fluency in Spanish, both written and spoken
• Ability to work in a fast-paced environment and multitask effectively
