A company is looking for a Care Coach I to empower caregivers through support and education.
Key Responsibilities
- Engage and educate caregivers using phone and digital technology to enhance their well-being
- Provide support for caregiver programs, focusing on resource navigation and care coordination
- Document coaching interactions and identify caregiver challenges to provide appropriate resolutions
Required Qualifications
- 2+ years of experience in care/disease management with complex populations
- Knowledge of Medicaid, Medicare, and Managed Care systems
- Strong understanding of case management and community service delivery systems
- Fluency in Spanish, both written and spoken
- Ability to work in a fast-paced environment and multitask effectively
