A company is looking for a Care Coach I to empower caregivers through support and education.
Key Responsibilities
- Engage and educate caregivers using phone and digital technology to enhance their well-being
- Support caregiver programs by assisting with resource navigation and care coordination
- Document coaching interactions and provide feedback for process improvement
Required Qualifications, Training, and Education
- 2+ years of relevant experience in care/disease management with older adults or individuals with disabilities
- Comprehensive knowledge of Medicaid, Medicare, and Managed Care systems
- Strong knowledge of case management and community service delivery systems
- Fluency in Spanish, both written and spoken
- Ability to multitask in a fast-paced environment
