Job Summary

A company is looking for a Care Coach I to empower caregivers through support and education.

Key Responsibilities

• Engage and educate caregivers using phone and digital technology to enhance their well-being
• Support caregiver programs by assisting with resource navigation and care coordination
• Document coaching interactions and provide feedback for process improvement

Required Qualifications, Training, and Education

• 2+ years of relevant experience in care/disease management with older adults or individuals with disabilities
• Comprehensive knowledge of Medicaid, Medicare, and Managed Care systems
• Strong knowledge of case management and community service delivery systems
• Fluency in Spanish, both written and spoken
• Ability to multitask in a fast-paced environment
