Key Responsibilities

• Engage and educate caregivers via phone and digital technology, empowering them to manage health and wellbeing
• Manage a caseload, assess needs, and assist caregivers in achieving care plan goals
• Document coaching activities and collaborate with stakeholders to improve care plans and identify solutions

Required Qualifications

• 4+ years of experience in care planning or case management
• Experience with pediatric populations is highly desired
• Comprehensive knowledge of industry concepts and principles
• Proven experience accessing community resources
• Ability to navigate and teach others to use mobile applications
