Job Summary
A company is looking for a Pre-Visit Care Connector to coordinate pre-visit assessments and provide care coordination via telephonic communication.
Key Responsibilities
- Conduct telephonic outreach and receive inbound calls from members
- Document demographic and medical history information accurately
- Ensure members have access to their Primary Care Provider and identify gaps in care
Required Qualifications
- High School Diploma / GED or equivalent work experience
- Minimum of 2 years of call center or telephonic customer service experience, preferably in healthcare
- 2+ years of healthcare experience or experience in social work/community outreach
- 1+ years of experience in collecting and documenting medical history information
- Proficient in Microsoft Word, Excel, and Outlook
