A company is looking for a Care Connector.
Key Responsibilities
- Supports and communicates with Members to identify needs and provide information on healthcare access and preventative health interventions
- Links Members to community resources and assists with scheduling appointments
- Maintains accurate documentation of Member interactions in the electronic care management platform
Required Qualifications, Training, and Education
- High School Diploma or GED required; Associate's degree preferred
- Minimum two (2) years of experience in a healthcare environment required
- Minimum one (1) year of telephonic customer service experience required, preferably in healthcare
- Medical terminology experience preferred
- Training or certification as a medical assistant, home health aide, nursing assistant, or similar healthcare paraprofessional preferred
