Job Summary

A company is looking for a Care Connector.

Key Responsibilities
  • Supports and communicates with Members to identify needs and provide information on healthcare access and preventative health interventions
  • Links Members to community resources and assists with scheduling appointments
  • Maintains accurate documentation of Member interactions in the electronic care management platform
Required Qualifications, Training, and Education
  • High School Diploma or GED required; Associate's degree preferred
  • Minimum two (2) years of experience in a healthcare environment required
  • Minimum one (1) year of telephonic customer service experience required, preferably in healthcare
  • Medical terminology experience preferred
  • Training or certification as a medical assistant, home health aide, nursing assistant, or similar healthcare paraprofessional preferred
