Job Summary

A company is looking for a Care Continuum Partner to act as a liaison between patients, families, and clinical teams throughout the care continuum.

Key Responsibilities

• Collaborate with the clinical care team to support pre-visit planning and manage patient flow
• Coordinate and schedule complex appointments across various care services
• Assess patient and family needs, ensuring care plan compliance and facilitating communication among care teams

Required Qualifications

• High School Diploma/GED with a commitment to advancing education
• 2 years of experience interacting with patients and performing core functions
• 3 years of experience in a complex health system environment or related field
• Working knowledge of basic computer functionality
• Ability to handle numerous tasks simultaneously and adapt to changing priorities
