Job Summary
A company is looking for a Care Coordinator I to facilitate client care and support clinicians in a mental health telehealth setting.
Key Responsibilities
- Facilitate the execution of clinician-developed care plans and assist clients with scheduling needs
- Support clinical teams with administrative tasks and ensure effective communication among team members
- Monitor patient progress, ensure follow-up scheduling, and maintain accurate client records
Required Qualifications
- Previous experience in a medical or mental health office, particularly with scheduling and referral coordination
- Working knowledge of Electronic Medical Records Systems
- Experience with Excel, Word, and other computer programs
- Ability to work collaboratively in a team with minimal supervision
- Customer service experience (2 years preferred)
