A company is looking for a Care Management Associate.
Key Responsibilities
- Place outbound calls to members with acute health conditions
- Educate members on their Care Management program and connect them with a nurse/care manager
- Provide a positive member experience and answer inbound calls from members
Required Qualifications
- 2 years of experience in a customer service call center environment with outbound calling
- High Speed Broadband internet connection (100/25 mbps) required
- Dedicated workspace for remote work during scheduled hours
- High School Diploma or GED
