Care Navigator

Job Summary

A company is looking for a PRN Care Navigator.

Key Responsibilities

• Process referrals to secondary care and maintain quality directories of specialists and community services
• Manage communication and documentation of referrals to ensure continuity of care
• Serve as the main point of contact between specialists, clinic physicians, and patients for external care needs

Required Qualifications

• At least 3 years of applicable experience, preferably in a Primary Care setting
• High school diploma or GED required
• Proficient in Google spreadsheets and/or Excel, as well as database management
