Certified Health Coach

Job Summary

A company is looking for a Health Coach (Fully Remote).

Key Responsibilities

• Educate members on topics related to health management and empower group coaching participation
• Assist members in engaging with healthcare providers and care management teams regarding their health decisions
• Document member interactions and follow up on personal action plans to identify additional support needs

Required Qualifications

• At least 2 years of experience as a Health Coach
• Health Coach certification is required
• Availability to support 8:00 AM - 5:00 PM MST or 7:00 AM - 4:00 PM PST
• Associates degree or equivalent combination of education and experience
• Experience in community health or public health is preferred
