CNA Licensed Care Manager

Job Summary

A company is looking for an Associate Care Manager.

Key Responsibilities

• Engage with patients through messaging, phone calls, and telehealth to support care plan adherence
• Monitor patient data and follow up with those who have not submitted readings
• Educate patients on using digital health tools and measuring vital signs

Required Qualifications

• Experience in care coordination, patient engagement, or remote patient monitoring (RPM) is preferred
• Background as a Medical Assistant (MA), Certified Nursing Assistant (CNA), or similar healthcare role
• Proficient with digital health tools, EHR systems, and telehealth platforms
• Ability to work independently in a remote environment
• Bilingual (Spanish preferred) is a plus
