Key Responsibilities
- Conduct comprehensive assessments of clients' health and psychosocial needs, developing collaborative case management plans
- Facilitate communication and coordination among healthcare team members while advocating for clients' needs
- Document case management services and maintain client confidentiality in compliance with ethical and HIPAA standards
Required Qualifications
- Current, active, and unrestricted Registered Nurse licensure from an accredited institution
- A minimum of two years of full-time work providing direct clinical care
- Home office provision with access to high-speed internet and reliable cell phone coverage
- Ability to maintain accurate records and prepare reports while ensuring confidentiality
- Proficient in Microsoft Office products
