Job Summary

A company is looking for a Health Coach (Remote).

Key Responsibilities

• Conduct assessments of members' health status to improve medication adherence and health outcomes
• Utilize motivational interviewing to engage members and promote behavior changes for optimal health
• Provide education and support to empower members in their healthcare decisions and integrate them into available programs

Required Qualifications

• 1-3 years of experience with Microsoft Office
• 3-5 years of practice experience in relevant fields such as Renal, Diabetes, or Primary Care
• Experience in Managed Care and Medicare
• Must reside in EST/CST time zones
• Hardwired internet connection required
