Health Coach (Cardiology)

Job Summary

A company is looking for a Health Coach (Cardiology) to support patient education and engagement in a remote setting.

Key Responsibilities

• Provide education on chronic disease management, particularly heart failure
• Establish and monitor patient goals through shared decision-making
• Collaborate with the clinical team to ensure optimal patient outcomes and maintain documentation

Required Qualifications

• Health coach certification from an accredited program; bachelor's degree in health sciences preferred
• Experience in a medical office or healthcare environment, with cardiology coaching preferred
• Experience with telehealth is a plus
• Proficiency in electronic health records and Microsoft Office applications preferred
• Bi-lingual fluency in English and Spanish highly preferred
