Key Responsibilities

• Conduct comprehensive evaluations of referred members' needs and recommend approaches for case resolution
• Coordinate and implement care plan activities while monitoring progress and addressing quality of care issues
• Utilize motivational interviewing and negotiation skills to engage members and promote healthy lifestyle choices

Required Qualifications, Training, and Education

• Bachelor's degree in behavioral health or human services is required
• 2 years of experience in behavioral health, social services, or a related field
• Preferred experience in case management and discharge planning
• Managed care experience is preferred
• Proficiency in standard corporate software applications and effective computer skills
