Job Summary

A company is looking for a Case Manager, Registered Nurse (Remote).

Key Responsibilities

• Engage with members telephonically to coordinate care and develop individualized care plans
• Provide disease management education and connect members with resources to achieve health goals
• Document care management activities accurately and collaborate with the interdisciplinary care team

Required Qualifications

• Active and unrestricted Registered Nurse (RN) licensure in Kentucky or compact licensure
• Proficient in Microsoft Office Suite and healthcare technology tools
• Access to a private workspace for remote work
• Minimum of 3+ years of nursing experience and 2+ years in case management or related fields
• Associate's of Science in Nursing (ASN) degree required; Bachelor's of Science in Nursing (BSN) preferred
