Licensed Care Manager

Job Summary

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop and collaborate on care/service plans with interdisciplinary teams to support member transitions
• Facilitate communication and services between members, providers, and resources to meet healthcare needs

Required Qualifications

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing
• 2 - 4 years of related experience
• Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required
• Weekend availability is required
