Job Summary
A company is looking for a Social Worker for the Quality Operations Call Center - Remote.
Key Responsibilities:
- Conduct telephonic outreach to meet quality metrics and production targets
- Perform comprehensive assessments and document findings in compliance with department requirements
- Coordinate community resources and provide support to improve Social Determinants of Health for members
Required Qualifications:
- Bachelor's in Social Work (BSW) from an accredited program
- Active, unrestricted Compact Licensed Social Worker (LBSW) in the state of work
- 2+ years of experience as a social worker in a healthcare setting
- Experience addressing Social Determinants of Health (SDoH)
- Proficient in PC software skills such as Microsoft Word, Excel, PowerPoint, and Outlook
