North Carolina Licensed Senior Care Manager

Job Summary

A company is looking for a Senior Care Manager (RN).

Key Responsibilities

• Develops and assesses ongoing care plans and collaborates with providers to address members' unmet needs
• Coordinates care between members, families, and care provider teams to ensure appropriate services
• Monitors member status and revises care plans as necessary to meet goals and needs

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 4 - 6 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
• Experience with high-risk pregnancy population is preferred
