Required Qualifications

• Current license to practice independently in Ohio
• Licensed Independent Social Worker (LISW), LISW-S, LMFT, LPCC, or LPCC-S
• Individual professional malpractice liability insurance policy
• Individual NPI number
• Reliable internet connection and video-capable device
