A company is looking for a Client Care Coordinator (remote in Oregon).
Key Responsibilities
- Document and complete assessments, developing individualized plans of care
- Coordinate with providers and community resources to support members' behavioral health needs
- Facilitate communication between members and their support systems, tracking progress towards goals
Required Qualifications
- Two years' experience in healthcare administration or case management
- Bachelor's degree in health/human services field or equivalent experience
- Qualified Mental Health Associate (QMHA) certification required
- Strong planning and organizational skills
- Technology savvy, comfortable using MS and Google Suite software products
