Job Summary

Key Responsibilities

Required Qualifications

• Active Registered Nurse (RN) license in Pennsylvania and New York
• 3-5 years of clinical practical experience
• 2-3 years of experience in case management or home health care coordination
• Ability to work independently in a remote setting
• Proficient with Microsoft Office applications
