Job Summary
A company is looking for a Pre-Visit Care Connector to provide telephonic care coordination in a remote setting.
Key Responsibilities
- Conduct telephonic outreach and receive inbound calls to complete Pre-Visit Assessments
- Document member demographic and medical history information accurately
- Ensure members have access to Primary Care Providers and identify gaps in care
Required Qualifications
- High School Diploma / GED or equivalent work experience
- At least 2 years of call center or telephonic customer service experience, preferably in healthcare
- 2+ years of healthcare experience or social work/community outreach experience
- 1+ years of experience in collecting and documenting medical history using medical terminology
- Proficiency in Microsoft Word, Excel, and Outlook
