Key Responsibilities

• Provide telephonic coaching and educational resources to participants managing chronic health conditions
• Assess participant needs and communicate with healthcare professionals for care coordination
• Document participant activities and coaching sessions in the case tracking software

Required Qualifications

• Nursing degree from an accredited college or university
• Active and unrestricted Compact Registered Nurse License
• 2+ years of experience in direct participant coaching
• Intermediate knowledge of Microsoft Office and database software
