Registered Nurse Care Manager

Job Summary

A company is looking for a Care Manager, Registered Nurse- Remote.

Key Responsibilities

• Engage telephonically with members and providers to develop and implement comprehensive care plans
• Assess ongoing care needs and coordinate patient education and resources based on member requirements
• Facilitate transitions of care and maintain quality documentation of care plan progress and outcomes

Required Qualifications

• Current multi-state compact Registered Nurse licensure; BSN preferred
• 3-5 years of varied clinical experience, with a preference for telephonic Case Management
• Proficiency in electronic medical records and Microsoft Office applications
• Training in motivational interviewing preferred
• Ability to work from a home office that meets HIPAA requirements
