Job Summary

A company is looking for a Clinical Care Specialist.

Key Responsibilities

• Coordinate and implement Chronic Care Management (CCM) services for chronically ill patients in long-term care facilities
• Create and manage individualized Care Plans, monitor patient census, and track vital health metrics
• Conduct care coordination and provide education to patients, their families, and the care team

Required Qualifications

• Registered Nurse (RN) with an active compact license in the applicable state
• A minimum of five years of nursing experience
• Experience with chronically ill patients and telephonic case management preferred
• Strong computer skills and reliable high-speed internet required
• Self-motivated and able to work independently
