Job Summary
A company is looking for a Case Management and Care Transitions Professional Development Specialist I.
Key Responsibilities
- Consult with leadership to develop, implement, and evaluate educational programs for Nursing and Social Workers
- Assist in the implementation of standardized components of education, including orientation and ongoing staff development
- Conduct annual competence assessments and performance improvement initiatives
Required Qualifications, Training, and Education
- Bachelor of Nursing degree preferred, with a minimum of one year of experience
- RN licensure required; enrollment in an accredited BSN program within two years for certain hires
- Prior leadership experience preferred
- One year of case management and/or utilization review experience preferred
- Case Management certification and a Master's Degree preferred
