State Licensed Nurse Care Manager

Job Summary

A company is looking for a Nurse Care Manager - Compassionate Care Program (CST or PST - Bilingual).

Key Responsibilities

• Build empathetic relationships with patients and families while providing education on diagnosis and treatment options
• Conduct advance care planning and goals of care discussions to assess patients' needs and preferences
• Coordinate care across multiple providers and ensure accurate documentation of patient care

Required Qualifications

• Current RN License is required; BSN preferred
• 1+ years of utilization management experience preferred
• 3+ years of clinical practice in an ICU, hospice, or palliative care role
• Bilingual (Spanish) preferred
• Experience with goals of care discussions and end-of-life care decisions required
