Key Responsibilities
- Supervise and mentor a team of Patient Coordinators, providing guidance and performance feedback
- Contact patients to review recommendations and ensure adherence to treatment plans
- Conduct medication reconciliation and ensure accurate documentation of patient interactions
Required Qualifications
- Active certification as a Medical Assistant or an active LPN/LVN license
- 7+ years of clinical experience in acute or ambulatory care, with at least 2 years in a supervisory role
- Must have a HIPAA-compliant home office setup with reliable internet
- Proven ability to work independently and manage a team effectively
- Consistent attendance and adherence to work schedules are required
