Job Summary

A company is looking for a Care Manager (RN).

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop and coordinate ongoing care plans in collaboration with providers and community resources
• Monitor member progress and revise care plans as necessary to meet goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
