Job Summary

A company is looking for a Lead Care Manager RN.

Key Responsibilities

• Oversees and reviews ongoing care plans and collaborates with providers to address member needs
• Provides guidance and expert knowledge to the care management team to ensure quality control and adherence to policies
• Monitors member status and outcomes, revising care plans as necessary based on identified needs

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 5 - 7 years of related experience
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required
