Key Responsibilities
- Support care coordination teams and providers in developing effective transition plans for members
- Assist with member transitions based on enrollment or care service needs
- Track and maintain transition metrics to identify trends and improve processes
Required Qualifications
- Bachelor's degree required
- 2 - 4 years of related experience or equivalent experience
- Experience in care coordination or health services preferred
- Knowledge of community resources and healthcare services
- Ability to work in a hybrid role with occasional travel
