RN Senior Director Care Management

Job Summary

A company is looking for a Senior Director of Care Management.

Key Responsibilities

• Develop and deploy a member-centric care program strategy
• Lead the transformation of telephonic case management to a mix of high touch and low touch solutions
• Develop effective Medicare Care Management Strategy to improve outcomes and STAR ratings

Required Qualifications

• Current RN license with a Bachelor's degree in healthcare management or related field; advanced degree preferred
• 7+ years of experience in healthcare administration with progressively increasing leadership roles
• Experience in Case Management and Disease Management
• Demonstrated capability in the development and execution of strategic initiatives
• Proven track record of improving care outcomes
