Required Qualifications

• 3+ years of experience in geriatric care coordination or related fields
• Bachelor's or Master's degree in Social Work, Gerontology, or a related field preferred
• In-depth knowledge of community resources and aging services
• Experience with long-term care insurance policyholders preferred
• Must be 18 years of age or older
