Social Worker (MSW)

Job Summary

A company is looking for a Social Worker (MSW) for the Sixty Plus Program.

Key Responsibilities

• Promote client and caregiver wellness for the older adult population
• Assist with access to appropriate resources and develop care plans for clients with complex needs
• Support older adults in achieving their aging goals

Required Qualifications

• Master's degree from an accredited social work program (MSW)
• Two years of experience working with the older adult population, including dementia clients
• LCSW strongly preferred
• Experience in community presentations and hosting workshops
• Knowledge of Medicare and Medicaid
