About the Opportunity
We are seeking a Vermont-licensed pharmacist for a short-term, 6–8 week engagement to deliver Comprehensive Medication Reviews (CMRs) for approximately 100 patients. This pilot program may lead to future employment opportunities, but the only guarantee at this time is temporary work.
This is remote, flexible, and rewarding work—ideal for a pharmacist who is passionate about patient care, clinical decision-making, and collaboration with healthcare providers.
What You’ll Do
• Conduct comprehensive medication reviews (CMRs) with patients
• Apply motivational interviewing techniques to support adherence and patient engagement
• Make evidence-based clinical decisions and advocate for patients to optimize care
• Collaborate with prescribers and other healthcare providers to close gaps in care
• Document interventions accurately and effectively using an MTM platform
What We’re Looking For
• Active Vermont pharmacist license (required)
• Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
• Prior experience in clinical medication therapy management (MTM)
• Strong communication and interpersonal skills
• Ability to confidently collaborate with providers and advocate on behalf of patients
• Experience with motivational interviewing and patient engagement strategies
• Self-driven and able to manage time independently in a flexible work-from-home setting
