Texas Licensed Care Manager RN

Job Summary

A company is looking for a Care Manager RN to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop ongoing care plans and collaborate with providers to address unmet needs
• Coordinate care between members, families, and providers, ensuring person-centered services

Required Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience
• RN - Registered Nurse State Licensure or Compact State Licensure required
• Preferred experience in Acute Care settings or direct clinical nursing phone queue environment
• Strong clinical assessment and critical thinking skills
