Texas RN Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop and update care plans in collaboration with providers and community resources
• Provide ongoing monitoring and support to ensure members receive appropriate care

Required Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• Registered Nurse (RN) state licensure or Compact State Licensure required
• Ability to work remotely from Texas, Kansas, Oklahoma, Nebraska, or Mississippi
