Job Summary

A company is looking for a Social Worker Coordinator - Population Health.

Key Responsibilities

• Maintain a caseload of patients and manage their enrollment in Complex Case Management programs
• Develop and implement care plans, collaborating with interdisciplinary teams to ensure patient-centered care
• Perform assessments, outreach, and resource management to address social determinants of care and improve patient outcomes

Required Qualifications

• Bachelor's Degree in Social Work (required); Master's Degree preferred
• Licensure as required by state of practice (required)
• 2-3 years of experience in acute care, home health, or case management
• Case Management certification, LSW or LCSW preferred
• Familiarity with community resources and ability to work flexible hours
