Job Summary

A company is looking for a Clinical Program Coordinator RN, Medicare / Medicaid who will provide care coordination and case management services to members.

Key Responsibilities

• Provide care coordination, case management, and care management services to members with complex health care needs
• Educate and empower members to manage their diseases through disease management programs
• Assist with managing member health plan benefits and improving access and quality of care

Required Qualifications

• 5+ years of clinical nursing experience
• Experience collaborating with physicians in patient care management
• Current unencumbered Registered Nurse License in state of residency
