Job Summary

A company is looking for a Care Manager, Transition of Care (BH).

Key Responsibilities

• Evaluate member needs through post-discharge assessments and medication reconciliation
• Develop care/service plans and collaborate with healthcare teams to support member transitions
• Facilitate education and resources for members and coordinate services with medical providers

Required Qualifications, Training, and Education

• Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing
• 2 - 4 years of related experience
• Wisconsin Behavioral Health license required
• Relevant certifications such as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN
