Case Management Analyst

Job Summary

A company is looking for an Analyst, Case Management (Remote).

Key Responsibilities

• Conduct annual Health Risk Surveys to identify member needs for their Individual Plan of Care
• Inform care managers of newly identified health or safety risks and coordinate care activities
• Support member engagement and advocacy to address care coordination needs

Required Qualifications

• 2+ years in behavioral health, social services, or a related field
• Proficient in Microsoft Office Suite
• Access to a private workspace for job requirements
• High School Diploma or equivalent experience required
• Associate's or Bachelor's Degree in behavioral health or human services preferred
