Job Summary

A company is looking for a Remote Health Coach to empower patients in managing their chronic conditions.

Key Responsibilities

• Provide one-on-one health coaching for patients managing chronic conditions
• Utilize motivational interviewing and behavioral change techniques to guide patients toward health improvement
• Educate patients on disease management, medication adherence, and preventive care while offering ongoing support

Required Qualifications

• Active health coaching certification (e.g., NBHWC, NSHC, ACE Advanced)
• 1+ years of experience in health coaching or patient support, particularly in chronic condition management
• Proven expertise in behavior change techniques and motivational interviewing
• Experience with remote patient care or telehealth is highly desirable
• Strong organizational skills and ability to work independently in a virtual environment
