Job Summary

A company is looking for a Health Coach RN - remote (PA/NJ/DE).

Key Responsibilities:

• Provide health coaching and case management services to members across various health stages
• Develop and coordinate care plans while engaging members in self-management and lifestyle improvement
• Facilitate communication between patients, families, and healthcare teams, ensuring quality and cost-effective services

Required Qualifications:

• Current, active, and unrestricted Registered Nurse licensure in New Jersey and Pennsylvania
• Three years of experience in clinical, case management, and/or disease management
• Ability to work flexible hours
• Basic computer skills
• Strong clinical knowledge
