Certified Medical Assistant (CMA)

Job Summary

A company is looking for a Certified Medical Assistant (CMA)/Remote Care Coordinator in the Pacific Time Zone.

Key Responsibilities

• Conduct monthly CCM phone calls with patients and healthcare providers to ensure patient needs are met
• Create and revise personalized care plans in coordination with practices and providers
• Identify social determinants of health and collaborate with community resources to address patient needs

Required Qualifications

• At least 1 year of experience as a Certified or Registered Medical Assistant with active accreditation
• Experience with multiple EMR/EHR systems and familiarity with Microsoft Office and Google Suite
• Highly self-motivated and organized, capable of working independently
• Experience serving a geriatric population and/or in case management or care coordination is a plus
• Must maintain a professional demeanor on the phone with a compassionate tone
